Student Insurance Farmingdale State College Both the College and its students have mutual investment that should be protected. Adequate health coverage can serve to keep students in class and protect students and school against attrition due to the potential financial hardship caused by incurred medical expenses.
All full time students are automatically enrolled in this insurance plan. Premium is added to the College Tuition and fees, unless the student shows evidence of comparable insurance coverage by completing a waiver form by waiver deadline. A completed waiver form is required at each semester.
All full-time and part-time commuter students may enroll in this plan by completing a form request to the premium added to the students tuition and fees by the enrollment deadline. The school plan is serviced by Student Healthcare Solutions and underwritten by Nationwide Life Insurance Company and administered by Consolidated Health Plans, Inc. (CHP).
Students may enroll dependents on a voluntary basis by completing an online enrollment form on the website link referenced below by the enrollment deadline dates.
